Customer Care for your Equifax Credit Report

There are three ways you can request a free copy of your Equifax credit report, also known as your consumer file disclosure:

Phone

1-800-465-7166.  After making language selection, say “disclosure” or press 1. This is an automated phone system which is available 24 hours a day.

Mail or fax

Download and complete this form, then forward to Equifax using the address or fax number listed on the form.
